Information and Forms for Clinics scheduled in 2017
Beginning in January 2017, the UHC will offer organizations that host low- or no-cost gelding clinics up to $100 per horse gelded.
Thanks to the DeWitt Fund of the Community Foundation for Monterey County, we will meet and surpass our original goals for this year!

GETTING STARTED – HOST A CLINIC:
How to Conduct a Clinic – This handbook explains the various steps involved in setting up a gelding clinic.
Funding Guidelines and Application Process – This document includes specific policies and procedures for organizations that wish to apply for funding.

Clinic Funding Application(due at least 30 days PRIOR TO clinic date). Please be sure to read the new insurance requirement in the funding guidelines document above before completing the application.
Release of Liability Agreement (for Vet Schools only – due 5 days prior to clinic)
Participant Registration and Waiver (due after the clinic) – This form MUST be signed by the horse owner.
Veterinary Statement (due after the clinic)
W-9 (due after the clinic)
