Information and Forms for Clinics scheduled in 2018
The UHC offers funding for organizations that wish to host low or no-cost gelding clinics for their area. There are two levels of funding available: $60/horse and $100/horse.

REQUIRED CLINIC FORMS (all clinics):

Clinic Funding Application (due at least 30 days PRIOR TO clinic date). Please be sure to read the insurance requirement in the funding guidelines document above before completing the application. Insurance requirement not applicable to Veterinary Colleges or Schools.
Participant Registration and Waiver (due after the clinic) – This form MUST be completed by the horse owner getting their horse(s) gelded at the clinic.

Clinic Insurance Grant- The UHC recognizes that there may be a fee required to add the UHC as an additional insured on your organization’s liability policy. If you are unable to host the clinic because of this fee, the UHC can provide up to $75 to help cover the expense.
